Photocopy of municipal birth certificate of the child.

Photocopy of proof of residence e.g. Ration card, Electricity bills, passport, Voter I.D. Card, House tax receipt.

School leaving/ Transfer certificate and previous years Report card.

3 Recent Passport size colour photograph of the child and both parents.
